SMILE Pro laser vision correction · Himayat Nagar

SMILE Pro: flapless laser vision correction

SMILE Pro is laser vision correction for short-sight, done through a small opening with no corneal flap. It aims to reduce dependence on glasses and contact lenses. Suitability is confirmed by measuring your cornea first, and results vary from person to person.

How it's different

No flap. Just a tiny opening.

LASIK creates and lifts a thin flap; SMILE Pro does not. The laser shapes a small lenticule inside the cornea, and it is drawn out through a 2-4mm opening. The surface incision is smaller than a LASIK flap. Dr. H. Siva Mohan Reddy performs the procedure.

About the procedure

What flapless means in practice.

Features of the procedure, stated as facts. Individual results and recovery vary.

Flapless

No corneal flap is created, so flap-related complications do not arise.

Dry eye

Fewer surface nerves are cut than with a flap. Some dryness after the procedure is still common and is treated with drops.

Recovery

The laser step takes under a minute per eye. Vision usually improves over the first few days, and this varies.

The ZEISS VISUMAX 800

SMILE Pro is performed on the ZEISS VISUMAX 800, a femtosecond laser made by Carl Zeiss Meditec. SMILE Pro is the name of the procedure on this platform.

Patient guide

SMILE Pro: what to know before you decide.

What the assessment and the procedure involve, in order, with the risks stated plainly.

Who it is for

  • Adults with short-sight, with or without astigmatism, whose power has been stable for about a year.
  • Eyes with corneas of adequate thickness and regular shape.
  • Laser correction is generally deferred during pregnancy and breastfeeding, because the power can shift.

What happens, step by step

  1. Before the assessment. If you wear contact lenses, the team tells you how long to leave them out so the corneal readings are accurate.
  2. Assessment.Your power is measured, including after dilating drops. The cornea is mapped and its thickness measured. The tear film and the retina are examined.
  3. Counselling. You are told whether SMILE Pro, LASIK, ICL, or no procedure is advised, and why.
  4. Procedure.Numbing drops are applied. The femtosecond laser shapes a thin lens-shaped piece of tissue inside the cornea, which takes under a minute per eye. The surgeon then removes it through a 2 to 4 mm opening.
  5. Going home. You rest briefly and go home with drops and written instructions. Bring someone to accompany you.
  6. Reviews.Follow-up visits are scheduled and the dates are written down for you.

Which tests are done, and why

  • Corneal topography and tomography.Maps the shape of the cornea, front and back. It is the main check for early keratoconus, in which laser is not advised.
  • Corneal thickness.Shows whether enough tissue would remain after the correction your power needs.
  • Refraction before and after dilating drops.Confirms your true power, which is the number the laser treats.
  • Tear film assessment.Existing dry eye is identified and treated first.
  • Dilated retina examination.Short-sighted eyes can have weak areas in the retina that need to be known beforehand.

What you feel

  • The assessment tests are painless. Dilating drops sting briefly and blur near vision for a few hours.
  • During the laser step the eye is held steady by gentle suction. You feel pressure, and vision blurs or dims for some seconds.
  • For about a day afterwards the eyes can water and feel gritty.

Recovery, in days and weeks

  • First day.Rest. Vision is often slightly hazy on the first day. Do not rub your eyes.
  • First week.Use the drops on schedule. Avoid eye make-up, dust and swimming. Vision usually improves over the first few days.
  • First few weeks.Swimming and contact sport are resumed when the surgeon clears you, as written in your instructions.
  • First few months. Some dryness and fluctuation of vision can continue for weeks to a few months before settling.

Risks and limits

  • Risks include dry eye. Some dryness after the procedure is common and is treated with drops.
  • Glare and halos around lights at night.
  • Under-correction or over-correction, which may mean glasses for some tasks or a further procedure.
  • Interruption of the laser step if the eye moves and suction is lost. The procedure may then be continued, changed to another method or rescheduled.
  • Infection or inflammation, which are uncommon.
  • Weakening and bulging of the cornea (ectasia), which is rare and serious. The corneal mapping before the procedure is done to identify eyes at risk.
  • SMILE Pro does not treat long-sight, does not prevent presbyopia or cataract, and the tissue change cannot be undone.

The surgeon explains these, and how likely each is for your eye, before you consent.

What you are told before you consent

  • What the examination and measurements show, in plain language.
  • The options for your eye, including waiting or not having the procedure.
  • What the procedure is expected to do, and what it cannot do.
  • The risks that apply to your eye in particular.
  • A written estimate, and whether your insurance policy is likely to apply.
  • You do not need to decide on the day of the consultation. Nothing is scheduled without your signed consent.

What affects the cost

  • The technique advised for your eyes.
  • Your prescription and your corneal measurements.
  • Refractive procedures are usually excluded from health insurance.

A written estimate is given after the consultation, once your eyes have been examined.

What to bring

  • Your current glasses and older prescriptions, which show how stable your power has been.
  • Your contact lens details, if you wear them.
  • Earlier eye reports, if any.
  • Sunglasses, and someone to accompany you on the assessment day and the procedure day.
How care is organised here

Common questions

Frequently asked questions

What is the difference between SMILE Pro and LASIK?

In LASIK the surgeon creates and lifts a thin flap on the cornea, then reshapes the surface underneath. SMILE Pro is flapless, the laser shapes a small lens-shaped piece of tissue inside the cornea, which is removed through a tiny 2-4mm opening. Both are used to correct short-sight. SMILE involves a smaller surface incision, which may mean fewer dry-eye symptoms after the procedure for some people. Which is suitable depends on your eyes and is decided at your assessment.

Is SMILE Pro painful, and what is recovery like?

The eye is numbed with drops. Most people report pressure rather than pain during the procedure. Afterwards mild grittiness and watering are common for a day or so. Vision usually improves over the first few days, and recovery varies from person to person. You are given a written aftercare plan and follow-up visits.

Does SMILE Pro treat astigmatism or long-sight?

SMILE Pro treats short-sight (myopia) and short-sight combined with astigmatism. It does not treat long-sight (hyperopia). If you are long-sighted, LASIK or lens-based options are discussed with you instead.

Am I suitable for SMILE Pro?

It depends on your corneal thickness and shape and on a stable prescription. Thin or irregular corneas, keratoconus, or an unstable prescription can rule it out. We measure your cornea with topography and tomography. Sometimes LASIK or ICL is the more suitable option, and sometimes no laser procedure is advised.

Is the correction permanent?

The tissue change made by the laser does not reverse. Your eyes can still change over the years. For example, presbyopia develops in the forties and reading glasses are then needed, and a small number of people need glasses or a further procedure later. Results vary from person to person.

What makes SMILE Pro different from the older SMILE?

SMILE Pro is the version of SMILE performed on the ZEISS VISUMAX 800 femtosecond laser, which is the platform the surgeon uses. The principle, flapless correction through a small opening, is the same as in SMILE.

What does SMILE Pro cost?

It depends on your prescription and your eyes, which the surgeon explains at your suitability visit, once it is known whether SMILE Pro is suitable for you. You can ask about a suitability check on WhatsApp or by phone.

How do I arrange a suitability check?

Contact the hospital on WhatsApp or by phone. If you wear contact lenses, we will tell you how long to leave them out before the measurements so the readings are accurate.
